AMENDMENT NO 4 TO COMMITTEE AMENDMENT NO 1 PROPOSED TO

House Bill No. 1725

     AMEND after line 159 by inserting a new paragraph (g) with the following language:

          (g)  Copayments.  All individuals in the coverage group who are fulfilling the work requirement of this section shall be subject to the following copayments:

               (i)  Seventy-five Dollars ($75.00) for inpatient care;

              (ii)  Four Dollars ($4.00) for outpatient care;

              (iii)  Eight Dollars ($8.00) for nonemergency use of the emergency room (ER);

              (iv)  Four Dollars ($4.00) for a preferred prescription drug; or

              (v)  Eight Dollars ($8.00) for a nonpreferred prescription drug.

     The amount of total copayments provided for in this paragraph shall not be more than five percent (5%) of the individual's total income during a calendar quarter.  If this limit is reached, the individual shall not be charged for the rest of that quarter.

     FURTHER, AMEND the title to conform.
