AMENDMENT No. 1 PROPOSED TO

                             House Bill NO. 946

                             By Senator(s) Turner

     AMEND by striking the sentence on line 13 and substituting in lieu thereof the following:

The applicant shall complete the application and return it to the board, by mail or by electronic facsimile, before June 30. * * *  The renewal fee in an amount established by the board, but not to exceed Two Hundred Dollars ($200.00), a portion of which fee shall be used to support a program to aid impaired physicians and osteopaths, shall be included with mailed applications; for faxed applications, the appropriate renewal fee shall be mailed with a copy of the faxed application before June 30.
